Enhanced Care Management

The ECM program was implemented in January 2022 as part of the CalAIM (California Advancing and Innovating Medi-Cal) Initiative developed by the California Department of Health Care Services.

Enhanced Care Management is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of high cost and/or high-need members through coordination of services and comprehensive care management that is community-based, interdisciplinary, high-touch, and person-centered.

We are a Professional Care Management Service Organization

We offer services to the most vulnerable and highest-need Medi-Cal Managed Care Members in Kern County, California.

Engagement

Our ECM staff establish strong relationships with our members, most effectively through in-person interactions.

When in-person interaction is unavailable or does not meet the needs of the member, we use alternative methods to provide culturally appropriate and accessible communication according to the member's preference.

Care Plans

We develop a comprehensive, individualized, and person-centered care plan for each member within the first 90 days of enrollment. The care is focused on treating the"whole person", including physical, behavioral, and social needs.

Coordination of Care

Based on assessments, screening, and care plans, our Care Managers identify and connect members to needed community and social support services, including:
 - Housing
 - Transportation
 - Long-term care services
 - Employment counseling
 - Childcare
 - Food
 - Educational opportunities
 - Legal assistance
